What We Do

Queen Square Family Health Team is an organization that includes a multidisciplinary team of health care providers who offer comprehensive care for patients rostered/ registered with one of our physicians.

We are currently building our office space and will shortly be recruiting our interdisciplinary team:

Nurse Practitioners
Nurse Practitioners provide comprehensive assessments of patients: prevent, diagnose and treat illnesses or injuries, and help patients manage their chronic illnesses. We will have three nurse practitioners.

Registered Nurses
The Registered Nurses provide preventative health examinations, conduct health education, and monitor patients with chronic diseases. We will have a complement of 4 nurses and are looking to build a number of specialised programs.

Registered Dieticians
Dieticians provide advice on how nutrition and diet impact on maintaining health, as well as helping with dietary measures for prevention and treatment of diabetes, heart disease and other conditions.

Social Workers
Social workers help families by providing counseling /therapy or by finding and referring to community resources where and when needed.

Pharmacist
The Family Health Team pharmacist collaborates with all the members of the team to help optimize patients' drug therapy by identifying, preventing and resolving drug related issues. The pharmacist provides drug information to patients and members of the team through individual counselling or group teaching.

Occupational Therapist
Occupational therapists (OT) help people to be able to perform tasks in living and working environments. OTs can work with patients suffering from physical, developmental or emotional conditions, helping them to improve their ability to do physical tasks (motor functions). OTs help people to lead independent, productive and satisfying lives.

Chiropodist
Chiropodists work to maintain and improve patient mobility, to alleviate foot pain, and to care and treat problems with feet and the lower limbs. A Chiropodist can provide care to patients for a diverse range of conditions and problems: diabetic care; sport injuries; pediatrics; geriatrics; wound care and many others.
 

Our Admin Team:

Our administrative team currently comprises an Executive Director and a number of clerical staff. We will be adding a Human Resource Assistant in the future.
