Who we are:
Our team consists of nurse practitioners, occupational therapists, pharmacists and a specialist physician. We work closely with your family doctor to help support healthy aging for adults 65 years of age & older.
What we do:
Our goal is to help you age well by supporting health, safety and quality of life. Depending on your specific healthcare needs, you may be seen by one or more members of our team for:

- Medication review with our pharmacist (over a phone call)
- Fall prevention & home safety with the occupational therapist
- Assessment of memory and thinking skills
- Management of common symptoms in older adults
- Support for caregivers
- Link to community resources
What to expect:
You will be seen for two to three appointments by various members of our team. Each appointment will be between 60-90 minutes and scheduled on different days. During these appointments, we will assess risk factors that may affect your health and well-being. We will work with you and your family to address barriers to aging well. Queen Square FHT provides team-based care. As such, your electronic medical chart may be reviewed by team members in addition to your family doctor and health care providers providing direct care for the purpose of clinical case discussion and case management, reviewing treatment and intervention options, and seeking input, knowledge, expertise and resource support from healthcare providers to ensure the best quality of care.
How to prepare for your appointments:
Please bring the following (if applicable)

- Glasses
- Hearing aids
- All medications, including over-the-counter medications (like Tylenol), vitamins and supplements
- Completed forms (see attached)
A family member or friend who can provide additional information about your health.
A physician referral is required for this program.
